Dear clients, before going to the hospital, bring the following documents:
1- Insurance booklet (for insurance patients) if the insurance is online, have a national card
2- Identity card (for free patients)
3- Supplementary insurance letter (if you are not online with your supplementary insurance)
4- The principle of the doctor’s order for hospitalization
5- Presence of guardian (father or paternal grandfather) for people under 18 years old
6- The presence of the spouse with identification documents is necessary for consent to hysterectomy, curettage and closure of uterine tubes and ovarian cysts and all operations related to women.
Admission steps:
1- Referral to the admission unit and presentation of hospital documents
2- Refer to the hospital fund and pay the fee
3- Completing the admission process according to the provided guide sheet (taking the patient’s history by the doctor, performing radiography of the ECG, tests and seeing the patient by the insurance expert, etc.)
4- Being on the waiting list of the operating room and following the recommendations of the nursing unit colleaguesAttention:As long as the insurance expert is not present at the patient’s bedside to confirm the insurance and deliver the booklet, refrain from referring to the reception and receiving the patient insurance booklet separately. The first priority in the hospital is to keep the patient booklet for approval by the insurance expert.
Note: If patients are not able to submit a letter of introduction from the supplementary insurance for any reason at the time of admission, the necessary guidance will be provided at the time of discharge to receive a copy of the documents.

Dear clients, while wishing you good health and complete recovery and thanking you for choosing this center to treat your disease, note that providing a discharge order by your doctor does not mean your immediate discharge, and this process takes about 2 hours.

Note: Patients who are not seen by the insurance expert, their discharge will be postponed to the next morning after seeing the expert. Therefore, at the time of admission, be sure to deliver the booklet to the person in charge of admission so as not to encounter any problems.
